completed-sampled-spans — the built-in OTel provider is in use, so every
SAMPLED span that finishes reaches the diagnostic span processor. Unsampled
spans are absent because the sampler decides before any processor runs.
custom-provider — the application supplied tracerProviderFactory; a
custom host exposes start/activate/shutdown only and no completed-span
feed, so nothing is captured and nothing is fabricated.
noop-no-provider — no exporter is configured, so there is no provider to
observe.
unknown — the responder cannot describe the tracing stack: the connector
answering with no TelemetryPlugin registered at all, or a client answering
locally before any server exchange. Availability is reported, never
improvised.
